Hi Everyone!
I don't know if this is the right place to post,if it's already been done, etc.
My apologies in advance to Admin!

1.I have established if the casino I am requesting CB from is tagged.
2.Yes, I know my user name goes here in that box :)
3.I presume my total amount cash deposited WITHOUT bonus(I.E. 300%, etc)
4. The rest of this is now confusing....Multiple Questions!!!
Some account lookup at the casino may not show me all the details, how detailed do YOU need it, what if I enter the wrong amount accidently and banned from the Casino??!! The Horror, the Stress!
I'm joking...but not really.

I am a show me, don't tell me kind of Gal!!

Is there a sample form posted somewhere of how to redeem CB on your form? Step by step?
If so, Please tell me where.
If not, I would love to submit this as a suggestion.
it might help others out there, if any that are confused or intimidated by the details.

The fields are pretty self explanatory, really no need for a video. I am going to explain it to you step-by-step right now, I am sure you will get it right from me. :-)

For most casinos, the fields to fill the Cash Back Form are:

1. Casino Username > your username at the casino (it's impossible not to know it !)
2. Currency > the currency you use for your casino account (USD, GBP, EUR, BTC), you see the sign in your balance
3. Past Month (February 2018) Opening Balance > How much money you had in your casino balance at the start of past month (1st February for this example) at 00:01 hour, casino time. -- IF you don't know this info for sure, you must email to the Support Department (or contact Live Chat) of the casino from where you need this info and request it !
4. Total Deposits during Past Month (February 2018) > Same as above, IF you don't know this info for sure, you must email to the Support Department (or contact Live Chat) of the casino from where you need this info and request it !
5. Total Bonuses including VIP program, non-Cash Incentives & Cash Back from (Insert Casino Name) Casino during Past Month (February 2018) > Same as above, if you don't know exactly how much do you have received as extra incentives (this can be: bonuses, cash back received directly from the casino, redeemed loyalty points, extra spins awarded as a gift etc), then ask the Support Department (via email or Live Chat) of the casino from where you need this info !

Simply ask their Support Department exactly this very following quoted question: "Hi Support, my name is --insert Your Name-- and my log in details at your casino is --insert Your Username or email used to log in-- and I need to know exactly how much extra monetary value (please account for Free Spins gifts, Loyalty Points, Cash Back or straight deposit and no deposit Cash Bonuses, etc) I have received during the past month ?".

And once you have been given a numerical answer, you use that number to fill the required field on your Gambling City's Cash Back Form !
6. Total Withdrawals during Past Month (February 2018) > Exactly as above, IF you don't know this info for sure, you must email to the Support Department (or contact Live Chat) of the casino from where you need this info and request it !
7. Past Month (February 2018) Closing Balance > you need to know exactly how much money weas left in your casino balance on the last day of the past month at 23:59 hour, casino time. IF you don't know this info for sure, you must email to the Support Department (or contact Live Chat) of the casino from where you need this info and request it !
8. Comments or explanations to Gambling City, if any > You can almost always leave that field blank, unless you feel you need to mention something crucial.

Make sure you do not enter wrong values ! However, if it happens, contact admin@GamblingCity.com as soon as you did realized your error and explain in detail what was wrong with your Cash Back Claim submission ! (Then Gambling City might cancel your Cash Back Claim in cause and let you submit it again.) -- If you do enter wrong values in you Cash Back Claim form, most likely these will either be adjusted by what casino reports shows to Gambling City, or will simply be declined (most of the time) and you will receive no cash back for your wrongly submitted claim.

In your above post you mentioned something about "banned from the Casino", what did you meant by that ? -- If you meant to ask if you are still allowed to submit a Cash Back Claim to Gambling City for a casino where you just been banned, then the answer would be yes, you can still submit your claim ! However, if the casino will approve your claim or not and pay Gambling City the revenue for your activity before being banned is a different story, as it depends due to what reason/s did the casino banned your account !

So if you will be paid your Cash Back from Gambling City for your submitted Cash Back Claim request for a casino where you just been banned, might differ from case to case (and you might also want to take and discuss your case with Steve Russo from Gambling Grumbles by submitting a Gambling Grumble in case a dispute arise and can't be solved by yourself with the Support Department of the casino where you played).

PS: I don't think you will be able to find a more detailed step-by-step guide about how to submit a Cash Back Claim ! Had I made a video with everything I said in this post, would have been more confusing rather than en-lighting, I believe (just imagine watching someone sending couple emails on the video, or talking with the Live Chat support).
